Yep.  Lots of.  It would take a special kind of idiot that would use an
altitude hold when flying up a box canyon...  He/she likely would have
qualified for a Darwin award years prior...

Tom,
I'll admit the sample was a
poor one for OBAM aircraft but the condition can occur where topography
and
onboard distraction occur together.
