It is also possible to obtain a Sprint, or AT&T account and maybe even the 
BT account card, that allows you to connect to 1-800 in the US.

I use the Swiftcall Service, which allows you to buy credits in advance by 
credit card, and spend them each time you make an International call on your 
account.  US calls are very cheap, and the system is easy to use and 
cunningly devised.  When you wish to make an International call, you dial a 
London number...  You are prompted for a PIN, and told how much credit you 
have left... Then, you get a New York dial tone, and are able to dial out as 
if you were there!
