I am not intending to point fingers.  Please do not take offense:

Frankly, I'm shocked the inspector didn't find such a flagrant problem during 
the
normal build inspection and post build inspection process.

Wiring, hoses, cables, and mechanisms must be checked for proper clearances 
during
the build, after any maintenance and especially after a hose change.
One must anticipate that gravity works 24/7 and hoses and wiring must be secured
to prevent any conflict with any moving object, control or cable.   We have
all been bitten by this conflict between components and must carefully inspect
our work and that of others to prevent such incidents.

Attention to detail is essential.   A quick flashlight look around is not an 
inspection.
Good enough is not always good.  Of course, as Ira Rampil quoted one
of his professors "The enemy of good is better" and one can get too carried
away with overthinking a cable to hose clearance/securing issue when a simple
zip tie fix or making a standoff out of zip ties and plastic tubing/fairlead on
a cable can be done in minutes!  In my experience, a cable can be manufactured
in a few minutes.  If the cable run has not been done right, or there are areas
of concern with wires and cables near the rudder cables, cut the cable and
make a new one with some tubing for protection and just be done with it.  

Fred's comments are true.  Plastic tubing over your cables is neat, smart and 
prevents
inadvertent interference running cables around the fuselage.  I run cables
in tubing from the firewall, back to the pulley and then as far back as I
can in the rear of the fuselage.  Put an Adel clamp or plastic P clip on the
tube and never worry about the cable chafing or slapping around.  Clean quiet
and neat if done properly (i.e. run the cables straight and don't use the tubing
for bending the cable around items where a pulley is the proper answer.

Final Update on what caused the fuel leak in my Mono XS!

Below is the report from the LAA Inspector Engineer:

Found the fuel hose issue today, see pictures attached. The issue was the rudder
cable sawing away at the hose, it all gets very congested in the tunnel between
the seats. Where the hose goes in and out of the seat pan, there can only
be a 6 mm gap between the hoses and the cables. This is where the cable has cut
into the hose as the hose goes into a tee for one leg to the engine (fuel bleed
off return) - one to the rear mounted drain and the third leg to the tank
outlet connection. It was the last one that had the hole.
